Questions
Frequently asked questions
How do I find my immunization records?
Start with your state's immunization registry (IIS), then check with your doctor's office or pharmacy, and your school's health office if you were vaccinated as a student — these are the official sources for your history.
What is a state immunization registry (IIS)?
Most states keep a confidential database (an Immunization Information System) where providers report vaccinations. You or your doctor can often request your official record from it, even years later.
My child needs proof of vaccination for school — where do I get it?
Ask your child's pediatrician or your state's immunization registry for an official record. Many schools also accept records sent directly from the registry or the provider's office.
I'm traveling and need an official vaccination certificate — how do I get one?
Some countries require an International Certificate of Vaccination (the 'yellow card'), typically issued by a travel clinic or authorized provider after they administer or verify the required vaccines — it isn't something you fill out yourself.
Can I create my own immunization card?
No. Only licensed providers, pharmacies, clinics and government health departments can issue or validate an official immunization record. Fake or self-made vaccination cards are illegal and unsafe, and this site does not show templates or instructions for making one.
What if my old provider's office has closed?
Your state's immunization registry often retains records even after a provider's office closes. Your state or county health department can point you to the right contact.
Are digital vaccine record apps official?
Some digital record apps pull directly from official state registries or provider systems, while others don't — check with your state health department or provider to confirm an app's source is official before relying on it.
